TERMS & CONDITIONS

This is a contract between FurEver Friends, LLC and the pet owner whose signature appears below (hereinafter called the owner).

 

1. The owner represents that the dog(s) is/are in good health and have not been exposed to rabies, canine distemper, parvovirus, kennel cough or any other contagious diseases within the last 30 days and that all dogs attending FurEver Friends, LLC have received their vaccinations. Proof of vaccinations must be provided before the dog(s) is/are allowed to attend daycare.

2. The owner further represents that the dog(s) has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

3. The owner understands that he/she is solely responsible for any harm caused by his/her dog(s) while the dog(s) is/are attending FurEver Friends Doggie Daycare & Grooming.

4. The owner further understands and agrees that FurEver Friends, LLC and their staff and volunteers will not be liable for any problems that develop, provided reasonable care and precautions are followed, and the owner hereby releases them of any liability of any kind arising for his/her dog(s) attendance to FurEver Friends Doggie Daycare & Grooming.

5. The owner further understands that any problems which develop with his/her dog(s) will be handled as deemed best by the staff and volunteers of FurEver Friends, LLC at their sole discretion, and that the owner assumes full financial responsibility for any and all expenses involved. The owner hereby authorizes and holds harmless their veterinarian to share/release any and all information regarding the health and or treatment of their dog(s) before, during and after attendance at this facility, including follow up.

6. FurEver Friends, LLC shall not be held responsible for any injury, illness, theft or death of a pet that occurred while their dog(s) are in attendance of FurEver Friends Doggie Daycare & Grooming. I understand that when dogs are involved unexpected things happen and I assume all responsibility should my dog attack or be attacked by another dog or otherwise be injured. I also understand that all dogs handle daycare differently and that it can be a stressful situation and my dog may develop diarrhea or other illness/symptoms aggravated or brought on by stress.

7. The owner further represents that he/she is the owner of the dog(s), and shall pay any and all legal fees incurred by FurEver Friends, LLC should this be contested.

8. The owner agrees to pay the rate for services rendered while the dog(s) is/are attending FurEver Friends Doggie Daycare.
